For Patients & Caregivers

Our team of skilled doctors, nurses, therapists and home health aides keep the patient's health at the center of everything we do.

We create a customized care plan, based off of the patient's unique health needs, as MercyOne knows that not every patient faces the same challenges.

When Our Services Can Help

Home Care

Our home care services can help if you or your loved one:

  • Is managing a chronic disease like diabetes, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), or cancer.
  • Were recently discharged from a hospital, skilled nursing facility or medical rehabilitation facility.
  • Have had recent medication changes.
  • Have recently undergone joint replacement surgery.

Hospice

Assessing whether hospice is the next step can be challenging. Hospice can provide assistance when you or your loved one:

  • Struggles with pain management or other symptoms.
  • Needs frequent emergency care.
  • Faces increasing difficulty with daily tasks.
  • Experiences worrisome changes such as falls, infections, or respiratory issues.

Palliative Care

Palliative care might be the best option if you or your loved one:

  • Lives with a complex illness such as cancer, lung disease, dementia, or heart failure.
  • Experiences symptoms like pain, nausea, fatigue, or other distressing symptoms.
  • Requires additional physical, emotional, or spiritual support.
  • Struggles with managing care coordination.

At MercyOne home health and hospice agencies, we consider it a privilege to provide comfort during life's challenging moments. Our palliative care services are grounded in expertise and compassion, ensuring that respect for patients and families guides every decision.
